Continuing Care Services
Continuing Care Services are home-based services provided by Medik-Home’s qualified healthcare professionals. They are designed to provide the most appropriate care for patients with complex medical needs, categorized into four main areas. Each care plan is based on the patient’s needs and is developed in consultation with the patient and, if appropriate, a family member. The plan can be initiated by the physician during a visit or upon request. If the patient’s needs exceed our service capabilities, a hospital transfer will be promptly arranged.
Purpose
- These services address sensitive patient concerns requiring immediate attention.
- Includes: medical observation, medical stabilization, home medical care management, and home hospitalization at home.
1. Medical Observation at Home
In emergencies, this procedure can be used to stabilize a patient at home to prevent any further complications, with a maximum duration of 4 hours.

2. Medical Stabilization
Immediate Response

Rapid intervention to stabilize the patient's condition, and prevent further deterioration. Includes: Urinary retention, Nasal bleeding, hypoglycemia correction, Splinting, Cast removal and more

3. Management at home
For sick patients requiring immediate intervention, with more advanced care such as IV access, solutions, medication, Thoracentesis, Paracentesis, Nebulization, Dislocation reduction, etc. This care can last up to 24 hours and can be prolonged.

4. Hospitalization at Home
Hospitalization at Home (HAH) refers to medical care provided in the patient’s own home, rather than in a hospital. This approach allows patients to receive quality care from our experienced physicians while remaining in their familiar surroundings. It is suitable for more complex situations requiring extended care, up to 72 hours or more.

Hospitalization at Home (HAH) aims to provide personalized medical care directly in the patient’s home environment.
Purpose of Hospitalization at Home (HAH):
– Enable patients to receive compassionate care while remaining at home.
– Promote faster recovery in a comfortable environment, surrounded by loved ones.
Difference from traditional hospitalization:
– HAH takes place in the patient’s home, while traditional hospitalization takes place in a hospital.
– HAH offers a more personalized, less restrictive approach.
– HAH has less resources than Traditional hospitalization
Eligibility depends on the following factors:
– Nature and severity of medical condition.
– Patient’s Stability.
– Ability to provide the necessary care at home
Who is suitable for Hospitalization at Home?
First, the patient should meet one or more of the following criteria
General criteria
- Frequent hospitalization for same diagnosis
- Difficult to control physical or emotional serious medical illness
- Uncertainty regarding prognosis goals of care
- Chronic bedridden patient with limited mobility
- Patients family request for futile care or physician request
Oncologic criteria
- Advanced cancer progressing despite systemic treatments and functional decline
- Failure of appropriate Chemotherapy
- Uncontrolled pain
Convenience or recurrent criteria
- Multiple recent prior hospitalization with same symptoms or problems
- Long-term care patient with do not resuscitate and or comfort care
