Overview
The prices on this list reflect the cost of each service we provide. Please note that in some cases, tax for delivery may apply, including professional travel. If this is the case, it will be indicated on your invoice. There will be no unexpected charges. We guarantee transparency, however, our estimates may not be exact in relation to the billing notes, if only some tax is added or a service provided has a variable price in our price list. You should be able to verify your invoice easily.
Patient’s Financial Responsibility
The patient’s financial responsibility may vary according to specific needs, required services, and the selected payment plan. Payment must be completed by the stated due date; otherwise, late fees may be applied one week later. In some cases, a deposit or the full amount may be required in advance. To facilitate informed decision-making, we are transparent about our fees. If you are responsible for a patient who lives far from you, you can request a full report or visit our website.
Traveling charge
Travel charges are calculated based on the distance evaluated by GPS. We can locate your position, or you can send it to us. We then determine the distance between our location and the patient's home address, which may be a residence, hotel, or home care facility. This resulting distance, measured in kilometers, is what you will be charged. However, for accuracy, it will be verified once again at your home, and then matched with our price range.
Charges for service
For services like those from Medik-Pharm and Medik-Lab, our standard fee is 10-15% plus a fixed charge for technicians who have to collect the sample. If you have someone who can do this, you can give instructions. Once the sample is received, we send it to a lab or have it interpreted. We discuss how to proceed with the result as you prefer.
Potential tax
Please be advised that additional tax may be applied in the event of late payment, damage, or loss of material and equipment. For further details, please refer to the service agreement and billing note.
Payment terms
By accepting our services, you acknowledge and agree to the following payment terms:
- Prior to Moving :
- A deposit of 25% of the total cost is due or a fix amount (30-75) prior to any moving.
- This amount is non-refundable once the process has begun.
- Applicable Payment Methods:
- Direct transfer between local accounts.
- Transfer wire (bank account)/ express
- Credit card (not yet applicable).
- Cash payment.
- Other methods (zelle, Paypal in view)
- Refund in case of Special Circumstances:
- A 75% refund is possible in the event of death prior to the provision of services or if the services have not been carried out after the process has been initiated.
- Pricing terms:
- Rates are based on distance and travel.
- Here are the rates for travel:
- Less than 5 kilometers: 60-75 USD or equivalent in Gourdes.
- 5-8 kilometers: 75-85 USD or equivalent in Gourdes.
- 8-10 kilometers: 80-100 USD or equivalent in Gourdes.
- More than 10 kilometers: 100-150 USD or equivalent in Gourdes.
